Simple answer, yes to all of those especially in the US
Mostly NA. YGO from day one was advertised as a kids product. Especially so when 4kids got the rights to the anime.
But, some pieces have managed to slip through. I remember seeing Maiden of the Aqua for the first time. She dodged the reduction censorship for sure.
It's pretty much the US' fault. Yes, they are actually pushing for MORE censorship lately. I have no idea who has such a massive stick up their ass, but corporations and people pushing for all this censorship needs to chill out. Anime is being targeted once again on top of video games, and as someone that plays a lot of adult/NSFW games, there is a crazy amount of restrictions that happen on platforms like patreon.
In the 80's and 90's it was mostly Christian Conservatives. Nowadays, it's more the Progressive folks. (similar arguments for it too which is funny to me)

The it tends to be up to the parent company on what is allowed.
If you're making something (cartoons, comics, etc) for kids in the US you're not allowed to have guns, beer, or blood, if you're not then you're allowed to have whatever depending on whatever you're marketing it towards. Marvel and DC makes its comics for nobody so the ratings don't really matter for them
