I have seen some videos (most of them in reddit) no internet problems appear and also the message is that the player surrendered.
No, it's not bad internet, if you disconnect the game will tell you that you disconnect. The auto surrender op is referring to is a thing, you can google it and test it for yourself.
As for how common? Not that common, and it should not be a problem in low elo since the cheaters are using it to climb, since even though I know botting is possible to farm gems, I doubt it is prevalent yet. The bigger issue is the "reveal enemy card" hack since you would not know if your opponent is hacking or not. More people would probably just use this because it is undetectable.
Konami cares absolutely zero about exploiters and I have never once seen an instance of a cheater censured.

This has happend before in the past vs a Lab deck. Game was stuck in end phase for over a min then I got black screen with same message and was given a lose with no replay available.
Same against a Alba Zoa deck. He went first and did his stuff. Then game was stuck in main phase for a while until it black screened me again.
If it is a cheater then I wonder how many games have been robbed from someone who need to cheat just to win. If its a bug then its still bad cause your opponent get handed a free win.
Not saying it doesn't happen, but no its not a big issue.
