Do you have a process running in the background called Nahimic?
Are you using discord with experimental features on?
Thanks for replying! I checked, no background process called Nahimic. I tried ending processes like HP Audio Switch, all the Realtek audio background processes, etc. and that had no affect on the issue.
I also tried disabling the experimental features in Discord and even playing without Discord running at all - did not fix the issue.
Hm... Nahimic process was a long time an issue. And it helped for many ending the task. But if that is not for you the case, it is more difficult. Let us try some things ~
a) When you create a new character on a different save game slot, does the issue persists?
b) Does the issue persists when deactivating blutooth?
To avoid misunderstandings, would you share your DxDiag which gives us an overview of your PC and eventually indicators towards issues?

Since you checked on drivers, can I assume correctly you have the latest mainboard drivers for your laptop installed as well?

Thanks again. I tried a new save file and also turning off bluetooth - no dice, unfortunately.
I updated the original post to keep a running list of attempted solutions so far. Oddly, completely uninstalling Realtek HD Audio Drivers via Device Manager and reinstalling a driver I found online dated Feb 2022[www.intel.com] fixed the issue until I restarted my computer, after which the drivers seem to have updated automatically and the issue is back. Now I can't even seem to rollback again, though that might be for the best as I don't know how compatible those drivers are with other games.

Hm... I have definitely seen some similar issue with similar setup before. The biggest obstacle is that pseudo-hybrid-GPU where the game is especially sensitive with. But it should not cause a sound issue, should it?
The official approach would be to ensure a clean install of them using the Display Driver Uninstaller app, as mentioned in the pinned thread by the community managers.
What seems weird about your audio driver: I can see why you wrote it updated automatically. Yet it still looks weird, as the version which it has updated to, is from an older date than from the website you have linked. So the one from the website is clearly the better one aka the one intended to use by hardware support.
Oh, do not worry about DxDiag - I personally prefer if people do not randomly remove lines there, but I can understand the initial worries about personal information. It is just: the DxDiag never actually shares personal information which can ever be used to identify you. That is, unless someone name their PC like their own name. E.g. Mike Row named their PC in their network Mike Row Soft-PC.
In the DxDiag I see hints that your Laptop is definitely running on some outdated system software. I recommend to check the official HP support page for a manual update of your stuff. Why manual? Sometimes it is just necessary to uninstall and reinstall the stuff. In many cases it was the BIOS update which was missing. I definitely recommend a complete manual refresher except for the graphics drivers which are better downloaded from AMD/Nvidia.

Just unpack the zip in your game's folder, so that the content is in the same folder like your game's .exe
What is that good for? Well, I notice you had Xbox games installed. The Xbox software comes with something similar which runs also MHR. And it happens that they tend do be incompatible with each other, causing interference if you will.
The files inside the package makes it so that the game (MHR) has an easier time loading without affecting anything else. If it does not do anything for you, just erase the dinput8.dll and the reframework folder. They are harmless as they are. You just need to remember them when there is a new game update ~

Sorry for the reply delay and thanks a ton for sending this - I managed to *hopefully permanently* fix the issue by uninstalling my Realtek drivers and installing Realtek High-Definition (HD) Audio Driver, 6.0.8923.1 Rev.C. I have tried installing different drivers before so not sure why it works now, but I have played consistently for over a week now without issue.
Here's what's weird - my HP Support Assistant still tells me my current version of Realtek High Definition Audio is 6.0.9414.1. But I also upgraded to Windows 11 a few days ago (after playing with the sound fixed in Windows 10), so maybe that updated the drivers automatically without somehow breaking it again.
Either way, it still works, but I'll circle back if it crumples again. I'd recommend anyone else experiencing this issue consider completely uninstalling Realtek drivers and grabbing drivers manually online or seeing if Windows 11 somehow fixes it.
