What mods are you using?
Currently only HunterPie, but I have it disabled at the moment.
Just that and nothing else? What were you using before?
I was using REFramework, but I deleted the files but still cannot join a quest.
i wonder if it is related to mod usage of the host or simply a game bug
That is the crux of my questions. So far all pointers are indicating it is because of mods, since once mods are removed fully from all clients it started working again in those cases.
A glitch I cannot outrule. But with mods removed = working, it is usually an indicator that it is somewhat related.
Any attempt to join another Anomaly SoS did the exact same thing. 5 attempts and 5 fails. Restarting the game fixed the issue.
No mods ever installed. Was streaming, so checked Twitch's Inspector to see if there were any indication of connection instability but nothing on my end.
Do you recall the error codes?
The two codes thrown at the same time were [MR-0406] [MR-135-91].

The first one is suspected to be related to a mod issue. That one of the participants is using a REFramework script which glithces out and prevents from joining midgame. E.g. damage meter.
The second is a P2P issue, simply saying connection could not be established. That might be a full lobby or simple as that you could not connect with each other due to firewall, antivirus, dns etc.
