Why is this game so buggy?

This game started to crash repeatedly after I got about 20 hours in. Entering certain areas is causing these awful low-poly texture issues and then, if you talk to anyone or enter a home, the game crashes hard. This is ridiculous coming from such a publisher imho.

I've tried troubleshooting a number of ways, yeah. This has been an ongoing issue for years for this port of the game. People online are saying it's a large memory leak issue. Some download Special K and turn off texture caching or limit FPS through graphics settings, but those fixes did not work for me. It's frustrating.
