No. He has no personality of his own so he wears faces of various roles he fills in the family dynamic. The Old woman, for example, when he was in the kitchen helping prepare dinner, etc. is like a mother persona. From an article that summarizes it:

In the original film he does dress up like a lady and wear lipstick. When the brothers comes back to the house, he's wearing a wig and a dress and his lady face, and acting like a submissive house wife.

People over the years complain any time other films or projects show him cross dressing, but it's right there in the original too.

Oh boy here we go... So, uhh I'm sorry to be breaking it to you but he is most definitely not a transgender, far from it actually.

However he has a intellectual disability which is someone who has certain limitations in cognitive functionality. He essentially has autism and doesn't understand what he is doing other than what he is being told. His family are abusive to him, especially Drayton Sawyer (The Cook) who is abusive to everyone really. Hes the same towards Nubbins Sawyer (Hitchhiker).
