Supreme Survivor achievement

Please tell me, to get the achievement, do you need to beat the game in 1 go or can you go through each chapter separately? I’m also wondering if, for example, you died in chapter 4, should you start again from chapter 1 or should you only replay chapter 4?

You actually need to play the whole campaign from the start to the end without dying once. Therefore, you have to go back to the first level.

So 20 levels? Or a classic mod?

Yes, the main campaign . Classic mode doesn't count for to the Survivor achievement.
