In some areas there are environmental hazards that effect the boy unless you equip him with certain protective gear.

Is there any negative impact gameplay wise? I mean besides him complaining and then having a sickly animation?

You can't have him scavenge when he's complaining and also you're an awful father if you ignore him (can confirm based on my gf's commentary)
