Can't achieve 100% objectives on some missions in Campaign. Am I missing something?

Are there some optional objectives that I am missing in the Campaign missions or is this a bug. So far I have been pretty thorough with the missions, trying to explore every location and do every optional mission. At least as thorough as objective timers allow me to be. But in some missions, the objective progress does not go up to 100%. Specific missions for me are Mission 4 - 10 out of 14, Mission 7 - 5 out of 7 and Mission 9 - 11 out of 12. At this point, I have replayed Mission 9 three times. I made sure to do every single optional objective, I checked most of the map, the end mission screen even has 12 objectives marked as complete in it, yet the map screen shows only 92% completion.

Thanks for taking the time to bring this bug to our attention, I understand how frustrating it can be when you're aiming for that 100% completion, but some objectives don't seem to register correctly. I've shared this issue with the dev team and they'll be looking into it.
