A list of hidden teddy bears found by the community.
I think all the links reference the person/group that reported them first.

Bayou Paradis, Louisiana (Pre Patch 1.3)

Asylum The Fogometers (Pre Patch 1.4)

On top of one of the rotating fans.

Continue progressing through the level until you get to the top of the 2nd open area:

Jump to this little section and wait for a few seconds for the teleport.
The jump can be a little tricky, but its doable:

Temple of Prophecy (Pre Patch 1.4)

On top of one of the idol/chalenge thingys.

Go to the room with the baton teleport and the swinging hammer:

Jump onto the right side of the baton teleport thingy and jump to the left side of the planks in front:

Climb the plank and wait a few seconds for a teleport to the hidden teddy bear:

Temple of Blood (Nager) (Pre Patch 1.4)

Just chilling on top of an axe platform... But he's shy and needs to be properly 'invited' to show itself :) .

In the room with the lava pool and swinging axes. On the side that has two vases, climb onto one of the vases and then jump to the door frame. From the door frame jump on top of the rope:

Go to the far side of the rope:

On top of the offices area near the two padlock doors.

Asylum Gateway (Pre Patch 1.5)

From the teddy bear teleport point (front gate of the asylum), turn right into this area:

Jump into the invisible hole in the wall:

Asylum The Fogometers (Pre Patch 1.5)

In the same place as before, but there is no warp zone this time. Go to the room with one rotating fan (or whatever). Climb up to one of the fans and start walking against the center pilar, eventually Shadow Man will grab onto it like a blood waterfall:

Climb all the way to the top and the teddy bear will be there.

Bayou Paradis, Louisiana

This hidden teddy bear was moved in patch v1.3 or v1.31 (I think), its a little harder to get now.
This is its new location:

It only spawns in the night version of Louisiana, and when the inside part of the church is loaded

Go outside and take the same route as the previous version teddy bear, to the church roof.
Go to the right side of the cross, near the front of the church, and jump to the middle of the right side of the little roof slope:

If done correctly, this will make the game load the inside part of the church, while we are still outside. The character should slide to an invisible floor, on the right side:

Hug the church wall and go forward untill this symbol appears:

(Just equip a light source and it should be easy to spot)

This represents the end of this invisible platform. Just run jump onto the next invisible platform, where the table is:

Jump to the table and to the lowered corner of the church roof.
Once on top of the church roof, the teddy bear is near the bell:

One of the sides is grabable, but i haven't found a way to reach it (by legit ways), just yet:

Summer Camp, Florida

This one was moved to a harder location. Its now on top of a giant tree.

It all starts in the same location as before, jump to the door and then to the roof:

Instead of going to the right side (like before), jump to the hill on the left, and then jump across, to the hill on the right. Make sure to come to this spot, to load the next sector:

After the next sector loads, keep moving along the hill and jump on top of the lamp:

Keep going along the hill. There will be some invisible walls due to the trees. Move across the house roof until you reach the electricity pole. Then, just climb the pole:

Once on top of the pole, turn towards the giant tree and run (don't walk or you will fall into the void)

This will load the next sector, but its a little different this time. Once in the new sector, drop down and move towards the house in the back. Jump on top of the rocks, then jump to the house roof and then to this hill:

Keep moving along the hill, to the oposite direction of the house, until you reach a rock. The rock is climbable from the back. In order to get there just move through the trees below. There will be some invisible walls, but it should be fine:

Once on top of the rock, move towards the giant tree. The tree should be climbable at this spot

The teddy bear will be on top of the tree, enjoying the view.

Cathedral of Pain

In the 'jack 2' chamber, the teddy is behind the retractor teleport.

Jump onto the rail:

This will trigger a voice line, then go back to the gateway area. Go to one of the sides, there is a lamp that can be grabbed and climbed:

This will spawn a spinning platform right next to it. Jump onto the platform and go for a ride. There are a few places where the platform stops and waits for you to jump onto somewhere else, then it will move to another position and you have to jump back onto the platform. Do your best to keep up with it, untill you reach the other side of the gateways:

From here, on one of the sides you can see the teddy bear, chilling on top of a roof, just jump down and grab it:

The fence has a hollow 'area' on the top-left side.
Jump to the top-left corner or the 'door' frame and jump to the invisible hole in the fence (where I'm pointing):

Spend all the Voodoo power and attack the wall with the uncharged Baton:

After about 18 times and a loud scream, the hidden waterfall to the hidden teddy bear will activate. From here the way to the teddy bear is the same as before

Hidden wall near the rotating thingy, near one of the retractors.

Climb the slope, cross the ratating thingy and jump onto one of the pipes:

On top of the swinging hammer, in the baton teleport chamber.

Face the swinging hammer at one of its sides. Wait for it to come your direction, jump onto the wodden part and immediately jump to the top, to grab this ledge:

Climb up and grab the invisible ledge on the roof:

Then clumb up to the hidden room in the roof, the teddy bear will be there:

Asylum Engine Block

Temple of Blood (Nager)

To get this teddy bear you need to have the swimming gad.
In the room, where you spawn after using the teddy bear. You will have to climb up one of the statues in the back of the room! From the top path, on one of the sides (both sides work):

Walk to the ledge and on top of the light, in order to grab this wall ledge:

After climbing, go to one of the back corners of the room:

Jump to the statue's hand and climb it like a blood waterfall:

On the statue's sholder, jump to its chest (the chest sides are slopes and will slide you down):

From the chest jump to the other arm (Shadow Man will not grab the arm, either you land on top of it or you will be blocked and fall on the other chest side and slide down):

The teddy bear will be where both statue's arms meet. You need to have pressed all the buttons, in the swimming gad's platform room, in order for this "rod" to be down:

Wow you're on fire with these! I was investigating this area with the cage since it seemed odd but it wouldn't have occured to me that there was a hollow section of fence on the top left. Awesome :D
