In the context of the game, since you're their boss, and you're hiring them for their attributes as crossdressing performers, it makes sense for that to be the default from a professional standpoint. It's odd that you still use it during the after-hours dates, but in the cultural context of otoko no ko, some transgender people do partake in it as something distinct from presenting their personal gender identity. Some of the writers and speakers on the subject within the community are themselves transgender but view the act itself as a different kind of gender play. Since canonically the player character can be an avid fan of otoko no ko, he may simply be hedging his bets until they tell him what they prefer.
Alternatively it might just have been easier to write. Code-switching in real conversation is already a pain in the ass, I can't imagine having to remember it for a huge cast of characters.
