Combat/Cover [minor but annoying] and Radial Menus [Major]

I couldn't shoot someone with a shotgun but they could hit me when I tried to backup/run away? It's also happened with the knife. Something is going on with Combat cover/shielding..

Also, the secondary radial menu doesn't work on PC... tried removing items from quick slot so I could activate Vigilance to no avail. Seems to be a hard cap on combat-effects akin to savegame slots.

Can you remember which combat this was, and if the enemy had a melee weapon, which weapon it was?

That is odd - you mean you can't scroll to the secondary radial menu, or a third one?

1. random encounter; I'm only 3 so they had to've been melee.

2. Assuming I can, how would I do that?

1 - OK, I will give this a look. Was it in the wilderness (rocks and trees) or on a street with destroyed cars? (Or something else)

2 - You scroll the mouse wheel once. Here's an example. https://imgur.com/a/etb6hKb

Or are you saying you have more than 16 total items and abilities?

2. Ah-ha! that's a good Option to know and it works :D I was trying to mouse-pick and that did not. ;)
