basically ddlc+ is prequel+parallel events+sequel to ddlc that greatly expand its canon lore (through many new hints and secrets that require serious theorizing effort to understand the whole big picture of ddlc canon lore), and it also bring some side stories and extra artworks stuff.
also buying ddlc+ is one way to support the devs, since the original ddlc was totally free.
ddlc+ can't be modded though. if you want to play ddlc mods you have to use the original ddlc.
