I forgot if it has to be purchased or not, but once you do, its game over for everyone.
Have you been upgrading your stats? If you're into mahjong or think you could learn, once you have a bit of cash, do that. Save-scum at the easy or medium tables in Lullaby. The hard table in Modern is bugged, so do not play it unless you're going for the completion point (requires one win at that table). The medium table in Modern is fine, though.
Then you can head over to the Rizap gym and literally purchase XP items at the counter, in addition to items that not only grant you XP but empty your stomach so that you can go to restaurants (another very good source of XP).
Also try looking for keys and safes. The safes are marked on your map and the keys are labelled in the completion list as well as never very far from their safes. Some of that stuff is really good.
There's a skill in the Other Skills section called Fortune. We've upgraded that a bit and I make nearly ¥100K in some of my street fights. Also, we've been putting points into stuff like digestion so that I can eat a lot at restaurants, and Kiryu is basically unstoppable for all the XP and cash he has.
By the way, after winning the Battle Royale at Modern Mahjong, we got ¥1 million (¥500K at Lullaby), so mahjong definitely pays off. Once you have cash, you can make more in probably several ways but that's one of them, and then when you're comfortable you can stock up on health from shops. Remember that in addition to staminans and toughnesses you can buy food like sandwiches, or medicines at the gym.
