It is an item, uncommon I think. Honestly I do not remember seeing it lately.

I have seen it at some point. But obviously not when playing on a too high Madness Level to generate chests.
Though it is RNG, so maybe just bad luck

It is RNG based on the seed. There is a seed sharing thread on the official discord that you can skim for piggy bank seeds
