Dark Miasma you need either Mulakah or the Rope from an event near the hatch on the first map, and then go into the hatch, you should be able to figure it out from there, and then Merciful you need to kill some people in cocoons just before the spider lair in the green portal. i don't remember trustworthy. and Tadpole isn't in the game yet from what i heard. As for the items, bloodseeker is an item you get from a quest reward that starts with a purple book node at the lake in the first map.

Bloodseeker - treasure map quest. Starts at the lake at the first map, high rarity
Jeweler's Ring - the jeweler event on the first map, just buy his rings
I believe the rest of the equipment is currently not in the game
Dark Miasma - throw a pig into the bottomless pit. Need rope. Capture the pig and throw him into the pit in the cellar
Friendly tadpole - not in the game yet
Merciful - swamp map. Kill the people trapped in the cocoons without looting them
Trustworthy/Untrustworthy - baker's son questline. Starts early on the first map
Dislocated Jaw - trader event early in volcano
Frightened - throw a teammate down the bottomless pit in the cellar
I don't know where to get electroshock or melted plating

Melted plating: Fail an event with Heiner that would usually give you a burn card (for example the burning farm on the first map). Heiner will get melted plating instead.
Electroshock: One of the altars. I believe the naga altar on the swamp map where you could read the naga notes and get the pet. Fail a roll there and you should get electroshock.

[Bloodie]

Frightened means looking into the pit. Jumping/throwing a teammate does Panic Scream.

Eternal Candle is also the treasure map quest reward.
