Another thing to consider is whether the game and AutoHotkey are running at the same privilege level. Try to have both running as administrator.
An alternative if that doesn't work (or you don't want to/can't try it) is to switch your attack buttons from numpad. I use OP{}L for my attack buttons (as that allows using Right Alt with my thumb for Roman Cancel), though I believe most people with a similar control style use UIOJK or something around there.
If you have to worry about ghosting, this site[drakeirving.github.io] can be useful for determining what keys can be pressed together on your keyboard.
Also, if you actually use Right Alt, then you may have to turn off Alt+Shift switching keyboard languages. (At least in Windows 10, Windows key + Space does the same thing anyway)
