On a base level, they can either jump over and cross up which you need to block like any other cross up (ie. block in the direction that she came from or in other words, hold forward to block); she land and hit you with a low attack ( hold down/back to block), or she can hit you with a jumping attack before landing from the Tandem Top (hold back to block).
If you want to lab this further, as I mentioned, depending on what combo was done before the Tandem Top and the stage position, there are some situations where she is too far away to cross up with an IAD (meaning that you only have to block high or low), or she has to do the Tandem top too late to do land a jumping attack meaty (meaning you can jumping block all pressure and escape relatively risk free)
The rabbit hole goes deeper (e.g Tandem Top - jumping attack - air dash - jumping attack) but that's not something you are going to see much of right now so you don't need to worry about it for now.
You need to block from the side where Millia is, you can tell which side she will be in (Block Normally, if she dash for crossup then you need to block opposite direction)
Also only 2 of Millia's Tandem Oki are true (Septem or 2D), the other 2 you can safely jump or DP or 6P, lots of options (Throw or Savior)

If you are attacked simultaneously from both directions, if you block one hit, it'll auto-block the hits from the other side if she does not wait for your block stagger to end. There is more incentive to either attack before tandum starts hitting, or after tandum stops hitting because of that.
So if you block the first hit, just keep holding that block direction. You'll block any subsequent hits from the opposite side automatically. Once the block/stagger stops, any follow ups are when you need to adjust the block direction.
Knowing which direction is going to be the first hit, however, is where you have to guess.
Her presenting you with those 50/50s is exactly what her kit is about fundamentally. She is built around mixups, ie forcing you to guess between one outcome or the other. That's why her health and damage are low, to offset the power of those 50/50s as far as I understand it. She's like the bird version of Chipp lol.
