It''s still a fair fight after the initial sneak attack, then it's a 1v1 until someone dies.
Think of it this way, your're a new player joining a server, haven't had much time to gear up, all of a sudden you are attacked by someone who has been playing since day 1 with the best possible gear. They were easily able to find you as your name is visible on the map making you an easy target. Now do you think that is fair?
At least with the names hidden, it will be harder for a fully geared player to find you. Even if both players are equally geared, names being hidden brings more skill into scene, who can move around and find players better if not a random encounter.
Further with names visible, this also makes your base visible if you spawn back at base, now making your base easier to find. Especially crucial at this point in the game with no available base defenses to be made.
I guess the "friendly fire" option (also not called "enable pvp") is there to make pve battles more demanding, cause you have to look out in a boss battle not to hit, shoot your teammats ... but if you look for a good pvp experience, there are many games, but Runescape is none of them - and I think won't be in the future?
