Like-- I genuinely do not understand how they got this wrong, unless they actively have people who have never played runescape working on this game.
This has been my thought for my entire play time so far. It feels like they contracted outside devs with UE5 experience, gave them some basic Runescape info, and figured they would add the rest later.
I'm genuinely baffled by the current review score.

Its a good entry to the survival genre. The Runescape ascetic is nice and adds game recognition with a solid and respectable IP. But RS3/OSRS players expecting to find RS4 here will be disappointed. Although tbh, with the way this first island quest works. You can easily see maps serving as quest lines in the future.
Maybe we could explore some more Runescape stories as maps get released!
