Missing "Collector" Achievement

I´ve completet the game 100% but one of the Last two relics didn't count (i suppose). The Last Room before "Blighted Lord is still blue as if i don't have anything. But i do have the "Lost Heirloom" and the "White Earrings". It bugs me to have 37 out of 38 achievements unlocked. Any help?

How many items do you have in total?
I have the achievement unlocked with 34 items

It sounds like you still don't have "everything" from the room then. When you continue the game after getting Ending B, there should be a relic in the room other than the two you mentioned.

I counted 33 (32 right now cause i´m in ng+ and lost the aegis curio), i checked maps / guides and its allways 33 .. so whats #34 then?

Which one?

Blighted Phantom, which was added to the game with the 1.1 update in mid-August.

The Lost Heirloom is found next to the bench in that room after Ending C and gives Lily a non-Blighted appearance. The Blighted Phantom is basically the opposite: it's found next to the bench after Ending B, and gives Lily a Blighted appearance (the two can't be worn at the same time).
