Tentacles were already growing out of her head in my playthrough before that relic was added.

pretty sure it's just there to let you play as the fully corrupt lily from the ending cutscenes, Even at max level and game progression she doesn't look nearly as messed up as the relic makes her,

For me it felt like the last boss was harder with this item equipped?!

Is it some kind of hard mode?

Didnt saw no difference in her appearence.

She looks the same as she does in the bad end, where you purify the last boss without having the necklace. Or well it's how she looks after trying to purify the last boss. Because that's the only time her skin darkens and she has glowing red eyes.
