A "Wound" is the segments of your Health bar.
If you get +Wounds via trinkets or the Zealot skill your health bar becomes even more segmented despite being the same max hp.
For each segment that is empty or corrupted you get the buff.
Or visually speaking for each "segment" or wound of your health bar turn black you get a stack.
Or I'm very bad at reading.
The correct answer is that missing health past the wound markers counts, including corrupted health. So basically, the lower your health and the more wound markers you have the more bonuses you get.
