The amount that passes through increases as your toughness decreases.
The intention of toughness is to provide defence against the large amount of ranged fire, so, it will always block ranged damage until it is fully depleted.
Yeah, blocking and dodging.
I don't think the game points it out, but your block is 360 degrees, so, if you hear the sound that you're about to be hit in the back, you don't have to turn around, you can just throw your block up (or dodge) and keep doing what you're doing.
Also worth noting that dodge is less effective when your ranged weapon is equipped. With your melee weapon equipped a dodge will generally "break" the enemy's tracking as they swing. Doing the same with a ranged weapon equipped will not break that tracking, and if you're still close enough they'll land the hit.
