There are multiple relationships you can form and/or strengthen with other characters depending on your choices, so picking and choosing (or all at the same time is an option) who you want to have more content with can lead to different playthroughs. And if you are a completionist, there are quite a number of scenes with characters (mainly of the adult variety) that can only be unlocked through specific choices made in a playthrough.
This is a visual novel at heart, so if you're familiar with those you'll find the City of Broken Dreamers (also known as COBD for short) isn't too different. It has loads of beautiful CG (in my opinion) or static images, but unlike most other visual novels I've read it has a pretty hefty amount of animated sequences. They are mostly used solely for the adult content, but COBD also has a fully animated intro, character introduction scenes (which is like a camera pan that goes over a character to give a stylistic view), and some short action sequences here and there. But I don't want to seem like I'm trying to fool you if you're looking for almost a purely animated visual novel; COBD is still mainly composed of CG images, but reserves the adult content and some highlighted moments in the game to a combination of CG images and animation. Personally, I enjoyed the balance that COBD has.
Might be a little too early to say, but being with what content is currently available we are only looking at what looks to be the first half the story (I might be wrong on this, but to me it feels like the first half). So far your choices have a lot of impact on how each character interacts with you, and there are a few scenes where depending on your choices it can determine how the scene plays out, but in regards to how any or all of these choices will affect the ending is not really known yet. All I can say is this can affect the first two questions (average playtime and replayability) depending on your investment in the story and/or characters.
I cannot help but sound like a shill for this game, but I hope you end up giving City of Broken Dreamers a chance. Its overall story (so far) doesn't reinvent the wheel if you're familiar with this game's subject matter, but I personally think it's definitely one of the better ones I've gone through and I'm highly anticipating the future content.
