Without knowing where you've been, what you've done and or haven't done, where you currently are and trying to do it'll be difficult to say without blurting out a complete answer.
If you're doing what I just mentioned above, (pipe skootching to Kit wind) and the alarm is going off, I'd have to say you were ID'd by the camera in as you left the office or by someone from the Kitchen, if not then could be a game glitch. Or corrupted game cache files.
I noticed some glitches in the PC version of SCCT in a couple missions but not required restarts like in SCDA.
SCDA is the worst glitch wise whether Xbox or PC. I recently began playing SCDA on the X series X and it seems worse than it did on the 2001.
Phantom18433 didn't elaborate on the # of KOs prior to the pipe skooch, outside of the chauffeur none need to be KO'd and only 1 up to that point needed to be diverted, (to exit garage). If all were KO'd then there'd still be -0- alerts. There're 6 cameras between the start of the mission on the roof to balcony#2 where you skooch to the kitchen. If the BG at the garage XP was KO'd and the nearby camera saw him the alert would sound long before the player made it to the office.
The only camera that could provide an alert that late in the mission would be the one in the office leading to balcony#2. However as far as I can recall, the office camera doesn't have the angle to see the XP. One is safe once one is tight to the XP and begins the unlocking process. So either the BG in the Hlwy sees Sam as he exits the office or there's something wrong with the game.
As far as my experience with the Xbox version, SC1 is the least buggiest of the 1st 4 games. I did have some issues playing the steam PC version but not in the Defense Ministry, they were issues later in the game where Sam was able to take cover in the Xbox version but not in the Steam PC version, even though his light meter read -0- and he was stuck to a wall the BG and wall camera saw him
You're wrong. That's what Fangu was talking about, there's a known mechanic/bug in this game, that just automatically scans the previous area you passed for bodies, no guards need to be conscious for that mechanic, the game itself searches for bodies and sounds the "body found" alarm if it finds one that is not hidden in complete darkness. I've been playing the game lately & experienced this mechanic and had to look up what the f was going on because it's very frustrating, and not really a sensible game mechanic.

How many times a minute does a camera rotate from left to right and back again, (regardless of the particular SC game), once every 10 secs yeah? If either of the 2 BGs in the garage had been dropped anywhere within view of a camera an alert would be sounded before Sam could exit to the stairwell.
There're no cameras in the hallway between office#1 and 2. I admit I never hung around to find out if the guy that enters office#1 (with the library) ever leaves it before or after Sam exits the hallway to the garage or to office#2.
But as previously stated Phantom never mentions leaving any bodies anywhere. I rarely ever leave a body and when I do, I know from past experience that it won't be found by BG or camera. Also I've never had a body found by BG or camera so I don't know what the alert sounds like, if it's proprietary then it would also be obvious to the player. If Phantom had left a body helter skelter one could think a light bulb would go off for the player "CRAP" as soon as the alert was sounded.
Phantom also says he created a new profile and tried it again. If it had been a save glitch the odds of it showing up at the same place would be odder still.
