If you've been using a third-party client to play oldschool Runescape, as of November 2019 there has been a revision. Specific 'quality of life' aspects to such clients would give the third-party client user a significant upper hand versus the vanilla client user. This mixed in with the storage of account information when signing into these clients made account security muddied and JAGEX decided to make an official ruleset. You can find this official post by simply Googling it.
If you're using RS3 and have been banned for macroing when you did not macro I would suggest looking at the timestamp of the ban and checking your email's integrity. If you're using Gmail you should check login locations, however, if there was no hijack you wouldn't have had this happen.
Any software that instigates auto-clicking or macro keybindings, (that are usually commonplace in other MMORPGs) will be detected in-game by the bot detection system. I've never heard of anyone being "too efficient" but I have heard of people being banned because they used macro programs unknowing that they were macro programs. A 'macro' isn't necessarily a bot, it could be anything repetitive to the T that the bot detection picks up on. The biggest culprits are usually mouse or keyboard software like Asus or Razer which can set keybinds to auto-'click' in segments repetitively. Any program that interacts with the game and isn't your finger on a mouse is a 'bot' in the eyes of the program, (discounting overlays like ALT1).
OSRS can be played on Runelite without problems. Jagex has approved this client. I'm also not aware of any RS3 clients people use or if that is allowed.
Jagex has done countless false bans in the past. You can find plenty of examples, even live streamers making fresh accounts for a contest and getting banned by a "live" person, which later was over turned as a JMOD was in the stream to correct it. Their automated "detection" system has also been known to catch people for false bans. Lots of overturns on Reddit for those who were lucky enough to get a JMOD to look into it by winning the Reddit lottery.
Otherwise it could be possible if you're using any mouse macro tool, or AHK.
