What does difficulty effect ?
Games like Nioh 2 or Wo Long didn't have difficulty settings in the conventional sense. They had NG+ and so on for difficulty scaling.

How does it manifest in this game ? I started with mid difficulty to get the hang of the game and I'm willing to increase difficulty to hard if it presents some kind of rewards(better loot, more xp, etc.)

Honestly not sure myself, I went in blind on twilight unsharpened simply for the fun of the challenge and have nothing to compare my game play to as I do not watch steamers/content creators.

I would assume that the enemies damage and defenses increase, while we know for sure that you at least start off with less stats on unsharpened which means less damage from the player, but better loot, exp I have no idea . . . I am clearing in each zone before moving on and I am currently clearing out Yokohama what I have noticed up to this point is that mission/chest loot is fixed while trash mob loot is random, I get a fair amount of blues off of trash mobs and a few golds now and then.
Dawn (Easy):
This mode is designed for players who want a more forgiving experience, with enemies being less aggressive and taking more damage.

Dusk (Normal):
This is the standard difficulty, offering a balanced challenge with enemies that require careful strategy and timing.

Twilight (Hard):
This mode is for experienced players looking for a real test, with enemies being more aggressive, dealing more damage, and having more complex attack patterns.

Enemies dealing more damage is a given. I'm also wondering if they get more spongier. That's something I really hate. Game is challenging enough but my only concern is that though I'm missing loot or more xp.

So in other words you want to know if difficulties effects rewards?

higher difficulty provides a boost to luck, enemies have new attack patterns and are more aggressive. You take more damage and in general i'd say for the full experience just play max difficulty.

higher diffs give enemies more damage and health, more Ki as well, lower does the opposite, ive not seen any new combos between the three but they are a tad more aggressive on harder diffs,

On easy you can also add extra bonuses to help. i think you can turn on extra Ki and extra HP for yourself as well.

NG+ Is HARD from what i remembered on PS5... like.. oops i messed up once im at 20% hp now, sucky thing is its not like Nioh 2 where its easy to rank up and make ng+ feel like normal again really

So it gives better rewards ?
I don't mind enemies being more agressive and dealing more damage but the more hp part is where I start to have problems. Trash mobs having sponge hp pools is a huge turn off in skill based games like this.

Do we have an achievement for beat the game in hard ?

Twilight? Likely not, because you can change difficulty at will.

The higher the difficulty, the enemies have:
- more damage output
- larger health pool
- more aggressive behavior (attacks more often, defends accurately more often)

Many games have this difficulty achievement, and yet you can change the difficulty at any time; it's just that your achievement will be locked if you change it.

I went to see if there was one, but I didn't find one.
Yeah Somewhere before the end of act I switch to the hardest difficulty and I've gotten used to it. Enemies don't seem spongier(even if they have more hp it is negligible) but you take more health and ki damage.

why would anyone play on hard when all it does is just make enemies deal more damage and receive less

Cause it's harder. They also have more moves and are supposedly more aggressive, but that last one seems like a blatant lie.

Because I found the middle difficulty to be too easy. Twilight difficulty is quite good - most enemies go down really quickly, but bosses can be decently challenging this way, making the game a lot more fun.
