There is one a slightly spooky part toward the end, but it does not show anything graphic, and nothing really bad happens.
Conversation topics stick to small talk and the environmental problems on the island that you end up fixing. A few animals are sick or stuck in trash, but you get to help them. The only potential for upset that I can imagine are the part where a fire breaks out on the island and your character runs off into the smoke, you briefly play as her worried grandpa to find her, and maybe sensitive kids getting anxious about the environment in real life and needing reassurance that someone takes care of sick animals and they can help by not throwing trash into nature etc.
Several people in reviews have said that their kids love the game.
