They already did that with the original with an extended ending but that's it. You're not going to see anything brand new.

All 3 games had very different formulas for their endings.
ME1 - the ending is very linear with a few choices that decide some of the details.
ME2 - is considered to be the best ending, as it takes into account so many different things you do throughout the game, and then also takes into account the choices you make during the final mission. ME2 is the only one of the 3 that allows you to go from total failure, to total victory, and everything in between.
ME3 is all about having made the right choices in the previous 2 games, as well as finding all the assets spread out around the Galaxy in ME3. This offers you a range of bad endings, with only one hinting that Shepard might live.
Just completed my Insanity run of the trilogy just a few days ago using this mod. Also paired it with Citadel Epilogue. I personally like it, I feel it gives it an end to things, even if it is possibly lorebreaking to some.
ME1: choice to save or sacrifice the council
ME2: Destroy or hand over the collector base
ME3: What to do with the Reapers
That's just one detail. One choice at the very end of the game. When people talk about the ending, they mean the mission, not just one decision at the end.
There are many details involved in the ending. None of your team dies in the ending, in ME1. Meanwhile, not only can your entire crew die in ME2, so too can Shepard. With ME2, who dies can be due to choices you make early in the game, by not doing loyalty missions, for instance.
This is why when people were polled as to which ending they preferred, ME1, ME2, or ME3, they chose ME2 by a wide margin. If it was just about the final choice, I'm not sure people would even see much difference, because you are correct, that final choice is the same formula. So I guess this boils down to what each person means when they say the ending.
