Also, info about insanity 3 from Reddit:
"achievements are bugged.
Reload the autosave before the Saren fight in ME1 (as long as it's an Insanity save) and beat him again and it should pop.
Source: I had to do this to get Insanity III (and also had to do something similar in ME1 to get Paramour III to pop)
If it doesn't work, you might have messed up with when you first selected the Insanity setting in ME3."
So, the reality is this. You cannot get Paramour by romancing anyone through all 3 games.
It starts with ME1 Romance choices are:
1. BroShep = Liara or Ashley.
2. FemShep = Liara of Kaidan.
So, since you can't romance Ashley or Kaidan in ME2, there is zero ability to romance just one person in all 3 games and get the achievement.
The first time I got it, I romanced Ashley in ME1, and then Miranda in ME2 and ME3.
You will also want to romance Liara in ME1, and then switch to Tali, or Miranda in ME2. It unlocks unique dialogue with Liara. She knows you cheated. She is, after all, an information broker.
So, if you romanced her in ME1, and she went through the trouble of rescuing your body in between ME1 and ME2, it makes sense that she would keep tabs on you in ME2. I don't remember exactly how the mechanics for this work, however. Meaning that you can do LotSB before locking in a romance with Miranda or Tali in ME2, or you can do so after the Suicide Mission, meaning you would have had your romance scene with Tali or Miranda just before the Omega Relay. If I recall, it's just the act of romancing either Tali or Miranda that she won't like. In other words, the romance doesn't have to be locked in, if I recall. But I may be wrong about that.
I don't remember if you can romance her in ME3 after that, but she won't reconnect with you in the LotSB section in ME2.
FemShep Run: ME1=Liara, ME2=Garrus, ME3=Garrus.
MaleShep Insanity Run: ME1=Nobody, ME2=Tali, ME3=Tali.
I got the first 2 Paramour achievements on Liara as I had to reload the scene due to crashing after it the first time.
Just managed to get Insanity 3 by replaying both ME1 & 2 endings.
Insanity 3: replayed ME1 Saren fight from start then re ran the Suicide Mission in ME2.
Paramour 3: replayed all of the romance/trigger scenes in order (Liara before Illos in ME1, Garrus before Suicide Mission in ME2, & Garrus Citadel Date in ME3).
It's because the triggers for Paramour were set for the main game. LotSB is a DLC, which is the only place you can interact with Liara on this level. It is for sure unfortunate that the Paramour trigger was not modified by the DLC.
It's kind of odd that you can't romance one single person through all 3 games to get Paramour 3 in one play through.
Some people had Save Games on Nexus Mods, so you could get the achievement without playing a second time.
You should have gotten Paramour 2 by romancing Liara in ME3. Odd that it didn't trigger for you when you locked in the romance with her in ME3.
