For the squadmate powers you'll have to use a bunch of squad powers 25 times, so you'll have to switch around your squadmates a bit (but this also goes towards the "complete 5 missions with X squadmate so no biggy). Kaiden and liara specifically have a lot of the powers you'll need to use.
As for Insanity? Super easy, use quicksave whenever you can.

My advice for you:
Just remember that for the armor/barrier ones where you have to use it 25 times you actually have to deal enough damage on that ability usage to break it in order to get it to count. I'd recommend dealing damage with your weapons beforehand, so that you can break it once you use that ability. It's really finicky sometimes even when you land it. Often an indicator will pop up on your screen tracking your progress milestones such as 5, 10, 15, and 25 (something along those intervals). Like someone else mentioned you can reload a save where there's a lot of those particular enemies are in an area where you can farm kills off of them. This is an excellent way in Mass Effect 2, especially, for getting the 25 X break for the armor and the 25 X break for the barriers. Be very patient as this will take a long time to achieve these. Also, note that when squad mates use their abilities it counts towards the achievement. Check out that guide I previously shared with you to plan out all three games to ensure that you will have the required tech/biotic abilities whether that be through your Shepard and the companions you can bring along to get all the ability based achievements.

AI Hacking is the only skill in ME1 that specifically needs a target (other than the Mako, the transport pads on The Citadel or the hologram bot on The Citadel), and it has to be a target that can be hacked. Luckily there are a LOT of missions with Geth involved. So my advice for AI Hacking:
- Find a mission with Geth
- Go in with Tali, or you as an Engineer
- Save prior to engaging in a fight
- Use AI Hacking as much as possible - > for it to count the text under the enemy has to say "Hacked"
- Once Geth gone, reload and repeat fight
- Repeat till you get the achievement.
The other option is not grind like that, just keep on fighting with Tali (if you have her, or you as an Engineer if not) and remembering to use the skill as much as you can.
Just remember: Disable AI from using skill, or at least set it to Defensive only, if using Tali for this achievement (or any of them via normal combat). If AI is set to use all skills, they will use AI Hacking (and other skills) & put it on CD... and this does not count towards you getting the achievement.
