I may be wrong but I think it based on the amount of overkill. Things like barriers, armor, and shield gate work against you.

Hmmm you might have a point, but I remember from each of my classic ME3 playthroughs that headshot from standard Mantis on normal difficulty would make Cerberus Assault Trooper's heads explode in the Artifact gathering side quest.
Not really no. I've tested shotguns close range and Mantis on low health no barriers no armor Assault Troopers on lowest difficulty. No head's exploded thus far.
Heads exploding and even entire bodies are based on abilities/combos/weapons used and the amount of overkill/damage involved. So if the last time you played through was in a + game playthrough with a high level/geared character then playing through normally could give the illusion things were toned down as you aren't doing so much overkill/damage.
