Leng killed her somehow, or wounded her mortally before I arrived.
She succumbs to her wounds after the exchange with her father.
No, but romancing Miranda and breaking up with her can cause her to die, which is why I brought it up to rule it out.
Also, I had max or near max paragon at this point and succeeded the dialogue check.
Yeah, something is definitely up. I never had this issue on the original version of ME3 in several playthroughs. I would bother to look up if there's a way to check or edit the save file, but I was so close to ending the game I just pushed through anyways.

Yeah, I rocked pure paragon over all three games, so I talked them down easily enough without taking sides.
you just have to complete her loyalty mission in ME2. Whether or not you still have her loyalty in ME3 doesn't matter.
