Bug with Miranda in ME3 (Spoilers)
I've just completed Sanctuary and had the misfortune of losing Miranda, even though to my knowledge I did everything right:

1. Miranda was not romanced in ME2 and broken up with afterwards.
2. Miranda was loyal in ME2.
3. Miranda was warned about Leng.
4. MIranda was given access to Alliance intel.

This is to my knowledge every single thing that can go wrong with the mission and I made sure I was careful not to ruin it. Yet she still died even with the paragon dialogue choice with Henry Lawson.

Anyone else running into this obvious bug?

You spoke to her 3 times before Sanctuary, right?

How did she die? From a wound delivered by Kai Leng or from a pistol shot by Henry Lawson?

Romancing Miranda isn't required....

Never seen her die, did you have too low a paragon score?

Leng killed her somehow, or wounded her mortally before I arrived.

She succumbs to her wounds after the exchange with her father.

No, but romancing Miranda and breaking up with her can cause her to die, which is why I brought it up to rule it out.

Also, I had max or near max paragon at this point and succeeded the dialogue check.

Something didn't save if she died. Maybe her ME2 loyalty mission status was incorrectly imported as incomplete. (If her ME2 loyalty mission wasn't completed, Miranda will die in ME3 no matter what.)

Yeah, something is definitely up. I never had this issue on the original version of ME3 in several playthroughs. I would bother to look up if there's a way to check or edit the save file, but I was so close to ending the game I just pushed through anyways.

There is a way to look it up through Trilogy Save Editor.

Oh ok, in ME2 after you did Jack and Miranda's mission, did you get them to resolve their argument on that FIRST fight they have when you get back to the ship? If you sided with Jack first and then later on got Miranda's loyalty back seperately, that supposedly will still get her killed.

Similar with Tali/Legion and getting that Geth/Quarian allience.

Yeah, I rocked pure paragon over all three games, so I talked them down easily enough without taking sides.

Losing her loyalty because of Jack then getting it back doesn't affect her survival.
you just have to complete her loyalty mission in ME2. Whether or not you still have her loyalty in ME3 doesn't matter.

I think the same thing happened to me, I am replaying from a much earlier save to try the kai lang warning again, but it did happen to me as well (didn't romance, but loyal, near full paragon, warned about kai lang, talked to her 3 times on the citadel and gave access to alliance intel. wtf

I read it differently: if you side with Jack in ME2, Miranda dies in ME3.
