Ending Bug
I finished the game and left the island and then reloaded thinking I could kill the boss again and choose to stay on the island and have a save there. However upon killing the boss and making it to the final tight corridor you hold E to move through it doesn't let me access the final cutscene. Is that intentional or just an unlucky bug.

Is the staying on the island ending the only way to keep going? do i have to replay the whole game again?

Sounds like a bug. Did you try restarting and verifying game files?
If you just want to keep going, you can just get out of the cave the way you came in.

I'm having a similar issue. I died during the boss fight then saved realizing how much progress I would lose if I died again (before entering the door to the end of the game) so using a tarp I saved and then went to go finish the fight. Afterwards I chose to stay not realizing that was the option, reloaded the fight won again except no cutscene/ no ending.
