During the ending you have the option to either get on the plane or grab the artifact. If you grab the artifact then you stay on the island. Looks like you choose that artifact ending. If you have a save available I recommend you go back and see the ending if you get on the plane.

You can also watch it here. And yes. If choose to go on the plane and both Virgina and Kelvin are still alive, you get an ending that shows that you the player(Jack Holt) have gotten Virginia Puffton pregnant and are living happily every after.
However don't feel too bad either. You got to experience the game playing it blind and doing decisions as you would without forethought. Thats a great experience to have.
I've played through the entire game, collected most of the notes, gotten all endings read up on all the characters. And I haven't seen anything that suggests she is mentally challenged in any way. Kelvin on the other hand is a different story.
I don't like they made her basically a normal human thats just really rich at the end of the game though. Would have been cool if she could remain a mutant.

Personally, I think it's a splendid idea. Heck, if we can romance Cait and Piper (and whomever else gets you off) in Fallout 4, then romancing Virginia isn't at all wrong.

Seems heavily implied he was the one who got her pregnant and they are now starting a family. Technically there is room for interpretation, but this conclusion seems like the most likely one.
