Suggestion: Greg, the cannibal companion!
I came up with the idea of a cannibal companion as soon as I saw the horror they feel when they are low on health. Being able to spare your opponent gives you a lot of opportunities and ideas, but at the moment, sparing an opponent does not provide any benefit, and even causes trouble for the player, when the spared one comes back with their tribesmen.
Below I will be speculating on how a new companion can be obtained and what they will be need for. (Greg is a conditional name)

How to get Greg
After ten days of survival, any male cannibal, regardless of tribe or height, with a small chance after being spared, can befriend a player. Greg will be obtained once per game, killing him removes him from the story.
How does the friendship process begin?
Being spared, the potential Greg will flee before the next season begins. Admiring the strength of the player's "tribe", after a long time, Greg, having changed his worldview, will decide to abandon his previous tribal marks and will be standing near the player's base with a flag made of a stick and a white cloth. From that moment on, his home tribe will take him as an outcast. When the player gets closer he will give him a gift in the form of a piece of mutant armor. From now, the previously hated cannibal becomes part of the player's civilized society.
What is Greg's unique trait?
Unlike Virginia, who leads her life more independently, Greg considers the player's society as a tribe in which he must successfully fulfill his function. Greg doesn't know the player's language, so the player cannot give orders directly to him, and all future decisions Greg will make intuitively, referring to what the player has done before.


How will Greg fulfill his functions?
Being agile and smart, Greg will assist the player in melee combat and use different tricks against enemies. His default weapon will always be a sharpened bone, but the player is able to give him other weapons that will change his gameplay and the way he approaches killing enemies. For example, a heavy but deadly axe will force Greg to keep his distance after each attack to regain his strength for a new one, and a machete will force Greg to use a more stealthy gameplay, sneaking up and attacking enemies from behind.
What other functions will Greg perform?
When Greg is bored, he will watch other companions or the player, getting experience and unlocking new abilities. For example, if he sees Kelvin or a player carrying logs, following the example, Greg will try to help the next time by following the same command as Kelvin.
So what is Greg's problem?
Greg is ambitious but tires quickly and will often need rest. So seeing him resting is as much as easy as seeing him fighting mutants while holding a huge log at the same time
How will Greg's relationship with the other companions be?
At first, cold. Greg will be avoided, and the companions will be avoided by Greg. The Companions trust to Greg will grow over time. As they become friends, they will help each other and share items.
Role in the story...
Greg, as all the alive companions will appear in the cutscene with the Cube. Afterward, Greg and the rest of the companions will help the player in the battle with the final boss. Greg's further fate depends on the player's choice.
The player left the island
In this case, Greg remains on the island, hinting that his home is here. The player and Greg wave goodbye and Greg's further fate will be unknown. (if the sequel of sotf will refer to site 2, then the reunion with greg will be interesting imo)

Greg and the others stay with the player and continue to fight the demons.
Small details about Greg...
Greg will be seeking to find materials and sometimes make himself some armor. If the player doesn't take creepy armor from mutants for a long time, Greg will take it himself. (The player will be able to take and give the armor to Greg)
Greg's favorite costume is pajamas (random fact)
Greg will dance to his favorite music on the radio
Greg will take the player as an idiot if he gives him soup "Greg" to try.
Greg can be given a gps, where he will also have a unique icon with the first letter of his name, "G"

Neat idea, could justify the ability to have a companion follow you into a cave too.

I would make my cannibal companion be able to drink the vodka and get fall-down, barf-until-it-hurts drunk. I mean real projectile vomiting against enemies that melts their faces. But I wouldn't name him Greg; I'd name him Ralph.
