There's a little stick you put on the wall, look in the book. Then you get a head and stick it on.

Where can I get the heads? I have just the option for an cannibal head to pin on the wall. I wished to decorate my rooms with deer heads, rabbits, birds, mutants and so on. I killed some deer, but there was no head option?!

You have to cut them off the animals. Once you go to the stick with heads in your inventory you pick one, pressing "R" goes to the next section of your pack in case they aren't in the first part that opens. Haven't tried it myself yet.
