Are there any essential mods i need to play/run the game for a first time playthrough?

Agreed, the 227 patch is essential.

But I strongly advise against using Unreal-Evolution, there were problems with this mod before which would harmfully (and unnecessarily) alter your game configuration files, meaning you won't be able to join some servers on multiplayer.

The mod author was advised against this but just ignored everyone, and it requires an advanced user to uninstall the mod completely, or a re-install of the whole game to fix the damage that the mod does to your unreal installation.

I'd recommend mods/maps like:

Project Gryphon (and the official server)

Level-Up! RPG Co-op (has both EU And US servers)

Star 861 (map)

Hexephet (2 awesome maps by the same author as Star 861)

SP-Don'tshootthechest01 (map by GTD-Carthage)

Fifth Vortex (mappack about the temples of Ehactora)

Triamid Ruins by Turboman (3 awesome maps)

To name a few.

Is it 227i, because i see other patches on the website such as 227g or h?

yes. also there are several renderers to choose from. and yes, it's 227i.

hey i can't seem to find these options, im using opengl renderer but cant find the v-sync or fov option

There is only a v-sync for the D3D render. You can turn it on by going to the System folder of your Unreal Gold install directory. Find the Unreal.ini file and open it with Notepad. Scroll down to the section called [D3DDrv.D3DRenderDevice]. Under this you'll find an option called UseVSync. It's probably set to False by default. Just change this to True and it should work. Don't forget to save the file. For the FOV setting, you'll have to open another file (also in the System folder) called User.ini. In this file there is a section called [Engine.PlayerPawn]. Look for the option called MainFOV and change it to whatever you like. I think the default setting is 90. So if you are running a resolution like 1920x1080, you should probably set it to about 107. Again remember to save the file, when you are done.

is that only for the D3D render, is there any way to have vsync for opengl?

because i can't seem to get a solid 60 frames

You could try changing another option in the Unreal.ini file. Under the [WinDrv.WindowsClient] section, there is an option called MinDesiredFrameRate. I don't know if it works, but change it to 60 and see what happens.
