I've had to reread some of the tutorials just from things not making sense the first few reads.
That said, GetsuFumaDen seemed to have always been in this kind of situation if I understood it's history right.
Not a bug, this is intended. If you pick up 5 souls, which by your count was +to main+4, it turns into a soul memory and starts over.

I'd be ok with hp and potion switching spots, but not putting main on spot 2 and heal on spot one. That would simplify the game far too much considering you get a ton of souls per run.
the diminished returns on powerups makes having potion 1 the more sensible notion.
since you would need 4 souls to boost hp, you have to pretty much use stage one just for that.
main is clearly better than sub, so it shouldn't be before sub wep. Sub weps can be great, don't get me wrong - but the main weps are far more useful imo.
your suggestion I htink is a good balance.
Maybe they could make it like Gradius III and Parodius, players were able to choose from different sets of power-ups before going into stage 1. I use sub weapons a lot, Sub+>Heal>Main+>MaxHP+ also sounds good to me.
BTW, I think the healing position depends on its healing power, if it heals less and only has the effect instantly (not a potion you can save for later use), it might still be reasonable to be placed as the first option.
