You also might want to check if nvidia experience bla bla bla center has applied some funny settings to the game.
Did you recently install any Nvidia update?
Can you try running the game on your onboard gpu? Do you get the same result?
I have a HD4000 and can run in HD resolution on highest settings with stable 90 FPS on standard gamemods and maps.
Select high profile and edit by going into advanced panel. Search your cpu, disable core-parking.
If you want to be sure your graphics card performs issue free, install xtreme-g custom drivers as nvidia even launches drivers nowadays that crash when their own technologies / softwarecode gets turned on in games like fxaa.
Trying the game to run in opengl instead of d3d might be worth a try to localise the issue.
