edit: i treid to change the resolution from the ini file and the menus still look blurry, but in game it looks ok, not sure if this problem prevents it from looking better
e.g. setres 1920x1080
type:
setres 1920x1200 (or whatever you desire)
type:
fov 100 (for 16:9 screens fov 106)
The resolution changes instantly, the fov only changes the next time your spawn.
If that doesn't work, try deleting User.ini and UT2004.ini
The game will recreate those, but your settings will be reset. I doubt it'll work but it's worth a try.
put "Texture Detail" "Character Detail" and "World Detail" to Highest. If this doesn't fix it, you may have changed something in the ini that would override these settings; in that case delete your UT2004.ini and try again. Your menu textures also seem blurry, these normally should look crisp no matter what, so I do recommend deleting your ini.
Lastly, your video card's software can also override your UT2004 settings. If you have Catalyst Control Center or the Nvdia equivalent, check your settings there.
I have an Intel Graphic Card, so you can expect some crappy perfomance, so when i formatted my laptop, i configured a lot of ♥♥♥♥♥♥♥♥ around it, and in the Control Panel of Intel - 3D i left "Texture Qualitty" on "Perfomance", which caused the blurry effect on, apparently, unreal engine games, i just changed it to "Qualitty" and its fixed now, so thanks anyway
