Empowered Shrine of the Disciples cannot be found in Shrine upgrade path

I have finished the game and got all the 48 achievements but the Empowered Shrine of Disciples and the other building is still locked (and cannot be seen in Shrine upgrade path).

The issue supposedly stemmed from the follower giving the Quest for the Disciple and then dying before doing the ritual (theorized by another user in Discord).

Please help, I would really like to build those buildings.
