Light Necklace [Bug?/Help]

I'm posting to get confirmation that I'm not doing anything wrong here and that I truly have run into a bug; and if so to report said bug.
I'm going to put everything else under a spoiler to be safe as it can be argued that this is a spoiler.
So my issue is with the Light Necklace and the Sacrifice Of The Flesh Ritual not triggering Baal to appear. The Sacrifice ritual goes though as if I just did it on a regular follower. No, I haven't gotten Baal yet either. I do have Aym so its not like everything is broken with this "quest".
I believe my issue started with my initial follower not being Sacrificed but dying another way. To be honest I'm not sure how they died seeing as I didn't know about this till just the other day looking up how to get Relics I'm missing and then a guide for how to get the two Relics that involve Baal and Aym.
I initially resurrected the follower wearing the Light Necklace and then without leaving the Temple/exiting the sermon/ritual menus I immediately tried to sacrifice the follower. No Baal.
When this didn't work I assumed it was because I rushed the rituals. So just today I resurrected the follower with the necklace again. This time to give things the best opportunity to work I let said follower walk out and make their bed. I then took the necklace away and gave it to another follower who I just got and hasn't died yet. I then waited till the next day to attempt the sacrifice again. Sadly the same issue occurred.

I'm at a loss on what to try next. From the several guides I've looked up for this. There doesn't seem to be any requirements to get Baal to appear. I.E. - worshiper level, specific type of follower, etc.

I did try to use the in game bug reporter but as I had gotten before when trying to report a bug, I got an error stating the bug report failed.
Mods, if this is truly a bug. Feel free to move it to the Bug Reports section. I just left it in General since I'm not 100% its a bug or if I'm doing something wrong.

Any help or advice on how to get around my issue would be greatly appreciated!

This definitely seems like some kind of bug, as yes, you are not doing anything wrong, and your methods to fix it were likely what most people would have tried. As for how to resolve this, I don't know.

Glad to hear I'm not crazy!
I was able to get video proof of the bug too. Sorry for the low quality. Laptop was the easiest to record this on and I didn't feel like plugging it in. I go somewhat quick though all my followers but feel free to slow the video down if you wish to confirm I don't already have Baal or another Light Necklace on a different follower.

Edit: I also just tried going though the process with The One Who Waits but same result.

P.S. Is the bug report feature broken for anyone else or just me? As per above I tried last night and once before but I also just tried again on my laptop today and got the same issue.

Massive_CM and QA have yet to get back to me. Not complaining!!!
But I decided to dig though some of the files I sent, on my own and came across this lovely entry in my Player.log

This tells me that my bug isn't that Baal will not appear but that Baal has died but isn't being listed as a possible resurrection.
Whats weird is I don't remember ever getting Baal. I am almost positive Aym was the first I got as I distinctly remember the sacrifice of the follower and getting excited when something different happened. I also remember the follower being old and Aym then of course joining old and it annoying me a bit.
That's why I'm almost positive I never got Baal as I would have remembered it like I did with Aym. This is even more true when Aym's joined date was after Baal's (,"DayJoined":277,).

I'm an idiot.............
Baal was in my list of followers to resurrect the entire time.... How I missed them like 5x now, don't ask......... After finding the above I watched my video over again at .5 speed and that's when I saw them.
