about to buy Cult of the Lamb. Should I buy only game itself or with all DLC's?

would like to take some advice. is it like purely cosmetic or actually add some more gameplay time

The DLCs are entirely cosmetics, as all content comes from updates. What I would do is buy the game itself at first, play around for an hour or two, and then go back to look at the DLCs to see if any of its cosmetic unlocks interest you.

Yes, as mentioned they're all cosmetic stuff including the fleece that comes from the Heretic's pack, it's just a reskinned default fleece to look like the bishop's robes.
