Did I screw up my 100% achievement run?
So, I have to do Sozo's side quest to achieve the 100% of the game, but I sacrificed him like and idiot in the "find his followers in Anura" part of the quest, and after bringing him back to life, the quest shows as "failed". Does he ever gives me the quest again? Is this a bug while the quest is still available or I screwed my 100% run?

He will give you the side quest again after some time also when Sozo dies the seed you used to get him as a follower will spawn again at his corpse in Spore Grotto

Sozo has died several times in my game. i keep resurrecting him and he keeps offering the quest. But I am kind of stuck trying to figure out how many times I'm supposed to find the same follower, or what I'm supposed to do with the follower after protecting him/it from sozo.

Tell him no. You can figure out the rest from there.

hmm yea should have thought of that. but i guess I didn't think of it because usually refusing just causes a faith loss while avoiding the quest on the list.

You actually want him to lose faith or to dissent and be able to re-educate him. I personally put him in the stocks. Anyway, you basically force him to go through withdrawals and when he's done dissenting the mushroom on the top of his head falls off and he comes back to his senses and thanks you for helping him. You get an achievement for it called help sozo. I actually discovered this accidentally just because I had low loyalty with my cult. You can force this though by refusing to give him mushrooms and not have any mushrooms crops growing. You don't need to do the mushroomo quest at all. You don't get any achievement for it and all Sozo does is eat him and then later requests you to find another one again.

Oh, I don't know, I've had one mate with a follower and hatched a mushroomo... Sadly, Sozo did eat the kid when I thought he didn't count... But thanks to this thread, I will be sure to put him through withdraws.

The thing that got me is that I did what should have put him through withdrawal which is just lock him up for two months or more. But I must have accepted his quest and just not done the quest rather than refused the quest.

Seems like there should be multiple paths for this like accept the quest then ignore it for too long rather than it forcing you to do outright refusal. After all I think the evil cult leader would have no problem promising more drugs while plotting something else like prison.
