I'm asking this question because each character only has below one leaf on a private part so...

They are whatever you want. Even if you get the hints with quests that follower X is someone else's sister or brother it doesn't matter. You can have 2 "confirmed" same sex followers mate and have an egg.

All in all, they are practically genderless. Just strange organisms with anthropomorphic humanoid shapes.

I vaguely recall some follower rescue quests the quest giver refering to the follower i am supposed to rescue sometimes as their "sister"

Or maybe i remember something wrong but other than their gender is up to your imagination.

Heket and the cat mom are female but most of the main characters arer male; The followers are generally gender-ambiguous unless stating so in a questline

As everyone mention before only canon sex is set for some npcs. Thou I also was thinking about the leaf thing, but I convinced myself, that since they reproduce from eggs, they lack mammary glands, so they only need to cover their lower part

The fig leaf is for artsty laughs. There is nothing to show. The game is meant to be comedic with dark humour undertones.. overtones.. a lot of dark humour and its definitely not an anatomically correct 'experience'.
