Naked follower outfit?

I have all the outfits except the naked one. I saw in another thread you get it through the rite of flesh ritual but I have done it multiple times and still haven't gotten it. Is there a certain amount of times you need to do this or is my game bugged?

I have all the packs, it's not falling under dlc? You may be thinking of the Olympian outfit which is just naked but with muscles
