3rd eye of a witness?

I'm trying to get the third eye of the witness but no matter what I do, I can't seem to get it. Is there somewhere specific you are meant to go for this? I just went on two crusades for the first two, fought the bosses at the end, and had no issue. For whatever reason though, I can't get the third one.

What happens at the end?

At the end of what?

You get one eye from doing each section again once. One eye per witness and one witness per zone. If a boss has bandages on then you killed it already and it's the wrong zone.

The end when you try to get the drop from the Witness, like TamanduaGirl is saying, it's only one drop per Witness/Crusade.

Witnessess appear in a dungeon AFTER defeating the Bishop. If you're looking for the third one, that means you need to have already defeated Bishop Kallamar in Anchordeep, and then gone back on another crusade in Anchordeep to fight the Witness at the end. The eye should appear from the treasure chest after defeating the Witness. Remember to pick it up! It won't automatically enter your inventory
