You get a heart from doing sermons after a certain point.
You only get the one heart from eating. I don't know if that's intended or not but that's how it is, it's been discussed before. The description did lead me to believe I could do it daily but it's one heart from food till you lose it and then you can eat again after you loose it if it's the next day.
Infinite sermon hearts are just stupid, imho. Should be capped at 1.
It takes resources and effort to maintain the cult for days. I don't mind the bonus for keeping people alive and maintained.
I'm hoping they change the sermon thing or it's a bug they fix soon, seems kind of ridiculous to force a heart on you every day doing basic loyalty functions. Pretty much negates any challange on next crusade. Ended up going into final boss fight on second playthrough with like 8 or 9 hearts without realizing, wasn't really fun. Between the health demon, diseased heart from bishop heart unlock, ability to eat a meal for hearts, and the multiple heart tarot cards, game is already really generous with +health bonuses but at least those are all optional. Tying a shield heart to sermons is too much.
If your desire is for it to be more difficult, I suggest avoiding the sermon.
You don't need to do a daily sermon, and that will up your difficulty level while reducing the daily hearts.
I have been lately, just kind of lame I have to give up the loyalty and faith boost from sermons just so I don't inadvertently enter a crusade with tons of health I don't want if I tend to my little ones for a few days. Should at least make it an optional take, like add a pedestal in the temple that it spawns on so I can choose to take it or not. Just shouldn't be forced.
Option 2, enter an arena and immediately teleport home. Then go on adventure with zero temporary hearts
I could, I forget if I lose faith doing that, I have to see. Someone also suggested just give the enemies a few free hits on first room as well so might do that as well for now. Just seems silly I have to cheese the game just to stick with the chosen difficulty. Either way, I'll do what I can to circumvent it.

Follower meat food gives two diseased heart instead of the normal one blue heart. Unsure if any other meals grant extra hearts, but meals like grass and poop actually damage you instead of giving you hearts.
Yeah, workarounds aren't ideal, and hopefully these are things that are addressed in the free content updates being discussed for the future.
I believe being martyred drops faith, but I don't recall "teleporting home" dropping faith.
